This one day workshop will provide you with practical tools and strategies to understand and embed effective person centred goal setting and care planning. It includes:
- Effectively engaging clients and carers in GDCP
- Strategies to understand client’s needs and priorities and how to translate these into tangible goals
- How to develop and document person centred care plans
- Reviewing care plans efficiently and effectively
- An introduction to the organisational systems, tools and templates that support effective GDCP.
You will have the opportunity to practice a range of techniques to engage clients in the goal setting process and work through case studies to put these skills into practice.

Target group: This workshop is designed for staff who and are actively involved in assessments and/or developing GDCP with their clients. Team leaders, coordinators and managers who directly oversee staff responsible for GDCP are also encouraged to attend. There are no pre-requisite training requirements.
Catering: Morning tea, lunch and afternoon tea will be provided (please enter dietary requirements when booking your ticket)

Statement of Attendance: Participants will receive a statement of attendance at the end of the session
Fees and invoicing: Registration fees include provision of all training materials (including a copy of the GDCP toolkit) and catering. 10% GST and the Eventbrite booking fees are included within the ticket price. A tax invoice will be emailed to you following registration.
Cancellation fees: There is a $50 cancellation fee up to 28 days before the workshop, $100 fee for cancellations made 7-27 days before the workshop. No refunds will be provided for cancellations made less than 7 days before the workshop commencement.
