This 1 day program will focus on practical strategies and tools that support effective goal setting and person centred care planning for people with low literacy levels. We will discuss a range of ways that GDCP can be created so that they are relevant for people with variable literacy skills. This will include:
- Introducing the concept of goal setting and care planning to clients from different backgrounds (and those with limited understanding of goal setting)
- Creative ways to document meaningful care plans (including simplified GDCP formats, visual care plans and easy English solutions)
- Policies and Procedures to support a flexible approach to GDCP.
You will also be provided with a range of resources and examples of care plans that have been successfully created in a range of formats.
"I got some great tips about how to introduce goals and care planning in lots of different ways. I’ve realised just how important my introduction is to helping people who are not familiar with goal setting get involved"
"What a fantastic day! My ‘toolbox’ is brimming with practical tools and ways to help my consumers of all skills and abilities"
"Today has been so useful and an excellent reminder that what it really important is coming up with a solution that’s right for each client"
"You make it all seem so easy and common sense! Thank you for another wonderful session, I know these new approaches will allow me to engage our young people in a much more meaningful way"
