This program is suitable for adults.

Do you have a chronic health condition? A chronic condition is defined as any medical condition lasting six months or longer, for example asthmas, arthritis, cancer, obesity, diabetes, chronic pain and heart disease.

Come to this one hour information sessions to find out about our six-week course. The course is an evidence based course in being conducted by ACT Health. It addresses topics such as symptom management, including pain, fatigue and depression, communicating with health providers, physical activity, healthy eating, action planning, problem-solving and decision making.

This course is a 6 week course. It will be at Belconnen Library from Wednesday 19 October to Wednesday 23 November. All sessions are 10.30am-1pm.

Bookings essential

It is strongly preferred that you are able to attend the first session, and at least four sessions to gain the most benefit from this course.
