2018 recipient of The Sir William Kilpatrick Churchill Fellowship to improve the quality of care of older people in our health system - USA
The aim of my Churchill Fellowship was to explore how to improve the quality of care of older people in our health system. To do this I ventured to the United States (US) to explore a framework of care for older people that is evidenced based, causes no harm and aligns with What Matters to the older adult and their family caregivers. Through my exploration of this framework, I learned the essentials from imminent leaders, thinkers and authors of improvement science, implementation science, scaling and sustainability. Combined with lessons in leadership and partnerships, I now have established networks, resources and experience to share and support the improvement of care of older people in our health system.
David will discuss his learning and provide ideas on how to improve the quality of care for older people in our health system.
Dr Claudia Strugnell - Senior Research Fellow, Global Obesity Centre, Deakin University
Claudia is one of the Senior Researchers on the RESPOND Project. RESPOND stands for the Reflective Evidence and Systems Intervention to Prevent Obesity and Non-communicable Disease. RESPOND is all about a shared community response to support healthy children.
The RESPOND project is based on a systems thinking model which acknowledges that things that contribute to our health are often interconnected and occur at the same time.
Systems thinking processes assist communities to identify all the different things that contribute to poor health and consider actions that will lead to healthier communities.
The RESPOND project relies on the insights of the community to identify causes and what they would like to do to improve health outcomes for our children.
Claudia will discuss the outcomes from the first monitoring data collection in the Central Hume catchment.
Janet Symes - House Coordinator, Waminda Community House, Benalla
Janet is the House Coordinator at Waminda Community House. Janet has been instrumental in the coordination of health outreach services attending the Community House on the same day.
Janet will discuss the benefits and the impacts that the coordination of the health outreach services has had on individuals and the community as a whole.
