Why is Case Conferencing important?
Case conferencing is a more formal, planned, and structured event separate from regular contacts. The goal of case conferencing is to provide holistic, coordinated, and integrated services across providers, and to reduce duplication.
In our experience, there is confusion about when a case conference should be held, who should be involved, what the case conference should cover, and what outcomes can be expected from case conferences.
