Session overview

This half day workshop explores how a range of validated assessment tools and processes are used in community care settings to identify levels of functionality and wellbeing and to measure related client outcomes. 


What’s covered

 

•   Setting the scene – introducing and discussing functionality and wellness with consumers

 

•   Tools designed to assess function and wellness

 

•   Setting up systems to monitor and measure functional and wellness gains

 

•   Reporting on functional and wellness outcomes for clients.

 

Who should participate?

This workshop is designed for senior staff and coordinators working with Commonwealth Home Support Programme clients wanting to improve their levels of function and wellbeing through targeted short and longer term reablement and restorative care interventions.

About the facilitator

Glen Sorenson 

The founder of Age Communications, Glen works as an independent consultant with all tiers of government, industry peaks, the non-government and private sectors. As a facilitator, trainer, researcher, writer and resource developer, Glen has a thorough understanding of policy and program directions in community care, packaged care, consumer directed care, person centred approaches and disability support.  His training design and delivery includes competency based vocational training, training package development and train-the-trainer courses.

The Junction Neighbourhood Centre Inc.

 

The Junction Neighbourhood Centre (JNC) provides a diverse range of services and support to disadvantaged groups across local government areas in Bondi Junction (Waverley), Randwick and Maroubra. Operating for 40 years, JNC has continuously strived to provide responsive and meaningful services to meet identified community needs and seek to develop community participation in the development of these services.

As a basic introduction, The Junction Neighbourhood Centre Inc. is a not for profit organisation resourced through government funding from Department of Social Services, NSW Family and Community Services, local government, community grants, donation, and some sponsorship.

We provide specialist services for familiesolder people, people with disabilities and their carers; facilitate the recruitment, training and referral of volunteers for Commonwealth Home Support Program (CHSP) in the City of Sydney and eastern suburbs; work with the Aboriginal and Torres Strait Islander communities in the Randwick LGA, run social inclusion groups & classes for people who are socially isolated; and organise practical assistance with outreach services including financial counselling, tax help etc.
