Are you interested in transforming medication safety and quality in your aged care home? Would you like to enhance your multidisciplinary team and be able to provide more personalised service to your residents?
Then an aged care onsite pharmacist may be right for your home.
This new measure is a revolution for clinical pharmacy services in residential aged care homes. Funded by the Australian government, the program embeds a credentialed pharmacist onsite as part of your clinical care team. The ACOP can provide expert support and advice about resident’s medications as well as contributing to clinical governance and a culture of continuous improvement and quality, person centred care for your residents.
