Health professionals and aged care workers aim to provide care that is patient-centred and delivered in line with a person’s goals, beliefs and preferences, particularly as people age and become more likely to have multiple chronic conditions. Advance Care Directives and a Palliative Approach are essential for facilitating this specialised care. In recognition of this Decision Assist and Palliative Care Tasmania are delivering a series of free workshops to assist primary health care providers improve knowledge, skills and confidence in advance care conversations and implementing a whole workplace approach.

1 day workshop content:

Morning:

The fundamentals of Advanced Care Planning and Palliative Care: 10-12pm

  • Advanced care Planning and Palliative Care terminology
  • What are advance care directives and what do we do with them?
  • A whole team approach to advance care planning and palliative care, what is your role?
  • Communication – how do we have the difficult conversations?

Afternoon:

Advanced Care Planning and Palliative Care for health care providers; diving deeper: 12.45 – 3pm

  • What are the benefits of Advanced Care Directives?
  • What are your roles and responsibilities?
  • How can you communicate effectively about a person’s goals and values to inform practitioners about appropriate future medical treatment?
  • How can you incorporate ACD’s into your workplace and routine care?
  • What are the legal requirements?

These workshops have been created to meet a range of people providing different responsibilities of care and everyone is welcome to attend the session that most suits their learning requirements.
