FOR GPs, NURSES & ALLIED HEALTH PROFESSIONALS

This session will provide GPs, Nurses & Allied Health Professionals with an understanding of the palliative care approach to caring for the elderly, including identifying referral pathways, planning for home care and catering to the needs of older people approaching the end of life with mental health conditions.

LEARNING OUTCOMES

  • Identify effective referral pathways and information to ensure continuity of patient care;

  • Discuss the aspects of planning for home care in older people with palliative care needs;

  • Identify the needs of people approaching the end of life with mental health conditions;

  • Discuss how we manage risk and scrutiny of clinical practice in the elderly

  • Identify effective clinical medicine in palliative care, addressing issues such as ‘what should we treat?’
