Eastern Melbourne Primary Health Network (EMPHN) and Maroondah City Council

Eastern Melbourne PHN (EMPHN) is a new organisation established to increase the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and to improve coordination of care to ensure patients receive the right care in the right place at the right time.

We have an in depth knowledge of the catchment and its communities, and have a proud history of working effectively with GPs, primary care, health services, government and many others in the region.
